29 CFR 1910.421(d)(9): Planning of a diving operation did not include an assessment of the safety and health aspects of the emergency procedures: Pacific Ocean off the coast of the Silver Strand, 32.39.3962N/117.09.6871 (LAT/LONG): On August 22, 2018, employees were performing SCUBA diving operations to mark a sunken road line. Effective procedures were not considered in the event a diver needed to be rescued. A diver suffered fatal injuries after he lost consciousness underwater and could not promptly be provided emergency resuscitation nor removed from the water.
