29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about July 18, 2018, and at times prior thereto, the employer did not clearly and specifically outline the requirements for testing the machines to verify the isolation and de-energization of energy sources when employees were engaged in replacing injection molding nozzles. Employees were exposed to burn hazards from high temperature plastics.
