29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about June 19, 2018, and at times prior thereto, the employer was using all-terrain vehicles to transport employees off-road to the work location without ensuring that employees were thoroughly and effectively trained in protective measures and hazards to be avoided while operating or riding in the Polaris Ranger ATV, exposing employees to the hazards of ejection.
