29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative: a) Throughout Facility - On or about May 23, 2018 and at times prior to, the employer did not ensure that employees used effective energy isolating devices for the control of hazardous energy when performing service and maintenance activities on equipment, thus exposing employees to caught-in, struck-by, crushed-by, and fall hazards.
