29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about April 27, 2018, at 9901 NW 106th St., MEDLEY, FL, the employer failed to mark the truck to identify the front-end attachment (fork extensions), and the approximate weight of the truck and attachment combination, and the capacity of the truck and attachment combination at maximum elevation with the load laterally centered; thus exposing the operator and the employees in the work area to a struck-by hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
