29 CFR 1910.147(c)(9): Affected employees were not notified by the employer or authorized employee of the application and removal of lockout devices or tagout devices: a) Polish Department - On or about April 12, 2018 and times prior to, authorized employees did not ensure that affected employees were notified when maintenance procedures requiring lockout/tagout were completed and equipment was ready to be re-energized and restarted, thus exposing employees to caught-in and struck-by hazards.
