29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about February 28, 2018, in the beveling area of the main production line, the employer did not perform the following lockout/tagout steps: a. Informing employees of the methods of controlling energy; b. Turning off or shutting down equipment; c. Isolating machine using isolating devices; d. Applying lockout/tagout devices; e. Removing stored energy; f. Verifying isolation and deenergization.
