29 CFR 1915.12(a)(1)(ii): The employer did not ensure that spaces and adjacent spaces that contain or have contained combustible or flammable liquids or gases were visually inspected and tested by a competent person to determine the atmosphere's oxygen content prior to initial entry into the space by an: (a) William E. Strait - On or about January 19, 2018 employees entered spaces where odor of flammable gas had been detected and the spaces were not tested to determine the oxygen content prior to entry.
