29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not ensure that employees were protected from injury, in that a drainage trench on the back wall of the Vertical Grow room was not guarded by a cover or grate. The trench was approximately 5 inches wide and ran the length of the room. This condition exposed employees to trip hazards.
