29 CFR 1926.1203(b)(1): Before work begins, the employer did not ensure that if the workplace contains one or more permit spaces, that the employer who identifies, or who receives notice of, a permit space informed exposed employees by posting danger signs or by any other equally effective means, of the existence and location of, and the danger posed by, each permit space. A sign reading "DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER" or using other similar language would satisfy the requirement for a sign: On or about November 15, 2017, at the above addressed jobsite, employees were performing construction type work activities, to include permit required confined space entry activities (elevator pit), wherein before the work began, the employer did not ensure that exposed employees were informed that the workplace contained one or more permit spaces, by posting danger signs or by any other equally effective means, of the existence and location of, and the danger posed by, each permit space, and wherein an employee who was working in an elevator pit, was crushed by an elevator car and was subsequently seriously injured and hospitalized, and also resulted in exposing the other employees working at this job site to permit space physical hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
