29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is more than 4 feet (1.2 m) above a lower level by covers or guardrail systems: This violation was observed on or about November 15, 2017, in the plating area, employees were allowed to access and work from elevated platforms that had holes in the walking surface. Employees were exposed to trip and fall hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps that is taking to ensure employee are protected from tripping into or stepping into or through any hole.
