29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complied with recognized and generally accepted good engineering practices: a) Boiler room next to ammonia refrigeration engine rooms: The employer failed to document that the rooms immediately outside of the engine room complied with RAGAGEP, such as, but not limited to, IIAR 2 2014 section 6.7. An eyewash station capable of providing 15 minutes of continuous flushing and a safety shower were not provided in the boiler room outside of the GEA/FES ammonia refrigeration room, on or about 10/10/17. b) GEA/FES ammonia refrigeration engine room: The employer failed to document that the exit doors leading from the engine room to the maintenance shop complied with RAGAGEP, such as, but not limited to, IIAR 2 2014 section 6.10.2. The doors were not equipped with panic bar hardware and did not open in the direction of travel, on or about 10/10/17. c) GEA/FES ammonia refrigeration engine room: The employer failed to document that the exit doors leading from the engine room to the boiler room complied with RAGAGEP, such as, but not limited to, IIAR 2 2014 section 6.10.2. The doors were not equipped with panic bar hardware and did not open in the direction of travel, on or about 10/10/17. d) GEA/FES ammonia refrigeration engine room: The employer failed to document that the exit doors leading from the control room to the emergency exits complied with RAGAGEP, such as, but not limited to, IIAR 2 2014 section 6.10.2. The doors were not equipped with panic bar hardware and did not open in the direction of travel, on or about 10/10/17. f) Access platform for the VK-1 Evaporative Condenser: The employer failed to document that the configuration of the discharge header located next to the access platform complied with RAGAGEP such as, but not limited to IIAR-2 2014 15.5.1.5. The termination of the discharge was t-shaped and in the event of a release from the pressure relief discharge, ammonia would be directed horizontally rather than vertically above the access platform potentially exposing employees, on or about 10/10/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
