29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected by a competent person prior to each use for wear, damage, proper installation, and usage as appropriate: a) Builder Services Group, Inc. dba Mato at 3355 County Road 27, Fort Lupton, Colorado 80621: On or before September 22, 2017, the exposing employer did not ensure the personal fall arrest systems were inspected by a competent person for appropriate design installation and usage. Employees were installing insulation near the roof without adequate fall protection. The personal fall arrest systems were not inspected by a competent person prior to each use for wear, damage, proper installation and usage as appropriate. On September 22, 2017, an employee sustained serious injury when he fell approximately 24 feet to the concrete floor below. This condition exposed employees to fall hazards. Note "competent person" is defined as "one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them."
