29 CFR 1910.146(d)(4)(v): The employer did not ensure that lighting equipment was supplied to enable employees to see well enough to work safely and to exit the space quickly in an emergency. a) On or about July 19, 2017 or prior there to, Sulfuric Acid Tank #5: Entrants into the confined space were not supplied with lighting aside from helmet lights to visualize hazards in the tank. Entrants in the tank were unable to see a 4 to 6 inch deep puddle of concentrated 98% Sulfuric Acid underneath the tank entrance. NOTE: Abatement certification and documentation are required for this item.
