29 CFR 1926.300(b)(3): "Types of guarding." One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are - barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about June 22, 2017, at the jobsite located near 7727 Horatio Drive, Saint Louis, Missouri, the employer did not ensure the chain and sprocket were guarded on the IST Reel (Make: Ritec; Model: Motorized Cable Reel XXL; S/N: 81) mounted in the company truck (US DOT 171 4460; TVK 116). An employee sustained an amputation to the tip of the ring finger of the right hand while operating the reel.
