29 CFR 1910.147(d)(1): Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. a) 650 South State street, Dover, DE - On or about July 5, 2017, the employer failed to prepare or shut down the equipment prior to servicing or performing repair and replacement of the actuator located on chiller # 3. The affected trained employee attempting to de-energize the machine was not knowledgeable of the proper method of controlling the hazardous energy stored in the equipment.
