29 CFR 1910.146(d)(4)(viii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not ensure that employees properly used rescue and emergency equipment needed to comply with 29 CFR 1910.146(d)(9): On or about September 22, 2017, the employer had employees working within railcars, GLNX 24138 and NATX 364178, and the employer did not provide the necessary rescue and emergency equipment at each permit-required confined space. This equipment would include the following: a. Harness for each authorized entrant, b. Retrieval line/lanyard to be attached at the center of the entrants back near shoulder level, and c. Mechanical device/tripod outside the permit space to which a retrieval line can be attached.
