29 CFR 1910.119(h)(2)(iv): The employer did not develop and implement safe work practices consistent with paragraph (f)(4) of this section to control the entrance, presence and exit of contract employers and contract employees in the covered process areas. Employees engaged in operating, inspecting, testing, and maintaining activities on the covered process equipment in the Tyson Prepared Foods facility at 9 N Washington in South Hutchinson, Kansas were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility in that the Employer did not follow established procedures and control entrance into the facility by support personnel employees.
