29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tag out procedures. On or about May 5, 2017 in the maintenance shop, the employees of National Maintenance Services and those of DHL Supply Chain were not informed of each others lockout and tag out procedures. Employees are exposed to crushing, freezing, and electrical hazards while performing service and maintenance on equipment including, but not limited to forklifts and battery chargers.
