29 CFR 1926.502(b)(2)(i): Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used between posts, were more than 19 inches (.5 m) apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches (48 cm) wide. a. On or about April 20, 2017 at the above addressed jobsite, on the 11th and 12th floors,employees and subcontractor employees performing construction operations were exposed to falls of approximately one hundred twenty (120) feet and one hundred ten (110) feet created by the mid rails on the guard rails being at the wrong height and incomplete to protect employees from fall hazards.
