29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about April 10, 2017, in the feather building, the employer failed to ensure that a locking device was installed on the main pressure relief valve when locking out the hydrolyzer. Employees were exposed to the hazard of being burned by hot steam.
