29 CFR 1910.146(d)(5)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space to determine if acceptable entry conditions existed before entry: The employer is failing to protect employees from the hazardous atmospheres associated with permit required confined space entries. This was most recently documented on February 1, 2017 at 9100 West Liberty Drive, Pleasant Valley, Missouri 64068. The employer is not protecting employees during the process when requiring employees to break the plane several times throughout the tank cleaning without first testing the hazardous atmosphere.
