29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complied with recognized and generally accepted good engineering practices. On or about August 8, 2016, and at times prior to and thereafter, the employer did not document that the wet out process complied with recognized and generally accepted good engineering practices, including but not limited to NFPA 30 Flammable and Combustible Liquids Code, 2012: a) The employer did not document that ignition sources, such as but not limited to, static and electrical sparks, were controlled per 6.5.1 b) The employer did not document that operations involving flammable and combustible liquids shall be reviewed to ensure that fire and explosion hazards are addressed by fire prevention, fire control, and emergency action plans per 6.4.1 and 21.6.3.
