29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used: a) 9000 River Rd, Delair, NJ, 08110- BLH, Age Oven: The employer did not ensure that employees were protected from falling from the age oven at the unguarded ladder access opening, on or about 2/23/2017. b) 9000 River Rd, Delair, NJ, 08110- Sutton 3, Extrusion Press: The employer did not ensure that employees were protected from falling from the catwalk at the unguarded ladder access opening, on or about 2/23/2017. c) 9000 River Rd, Delair, NJ, 08110- Danieli: The employer did not ensure that employees were protected from falling from the catwalk at the unguarded ladder access opening, on or about 4/13/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
