29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) Woodland Manufacturing - SE Chang Machine: Employees were required to place their hands into the point of operation of the SE Chang Corrugated Carton Flexo Printing Machine. An employee suffered an amputation while cleaning the roll of the machine. Occurred on or about 12/16/2016. b) Woodland Manufacturing - EMBA Box Machine: An employee suffered crushing injuries when he was placing part of his body on or near pinch points while connecting the glue feeder to the EMBA machine # 634002. Another employee was performing set up operations on the EMBA Machine at the same time, which caused the machine to cycle while the other employee was exposed to pinch points. Occurred on 01/16/2017. c) Woodland Manufacturing - Back of the Building: Two Ryobi 10" table saws did not have guards to protect employees for amputations. Occurred on or 02/08/2017.
