29 CFR 1910.146(d)(9): When entry operations were conducted under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces: a) On or about 12/01/2016 in the production areas; where local 911 emergency services were identified in the employer's permit required confined spaces program, those 911 services were not notified prior to entry into permit required confined spaces in the establishment.
