29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:. a) Throughout the facility, on or about 11/29/16: Medical evaluations for respirator use was not provided for employees wearing full face air purifying respirators such as maintenance employees when working with formaldehyde. Abatement certification must be submitted for this item.
