29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) On or about November 14, 2016 the employer did not ensure that lockout devices were affixed to all sources of hazardous energy while foil press machines were being cleaned and maintained, including but not limited to hydraulic, pneumatic and electrical sources exposing foil press operators and mechanics to hazards associated with the unexpected release of energy.
