29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: Whittier Rehab Hospital, 150 Flanders Road, Westborough, Massachusetts; On and before November 7, 2016, employees were placing tape over switches when performing electrical work in lieu of deactivating and tagging exposing them to electric shock hazards from the 277 volt circuits.
