29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about October 19, 2016, employees were exposed to the hazard of free falling more than 6 feet and contacting lower levels while using personal fall arrest systems that were not rigged to prevent these hazards. Redhawk Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(16)(iii) which was contained in OSHA inspection number 1083632, citation 2, item 1, and was affirmed as a final order on December 22, 2015, with respect to a worksite located at 1732 Lake Charles, Vernon Hills, Illinois. Redhawk Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(16)(iii) which was contained in OSHA inspection number 1019933, citation 1, item 1, and was affirmed as a final order on February 20, 2015, with respect to a worksite located at 20585 W. Highridge Drive, Kildeer, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
