29 CFR 1926.1204(c)(4): The employer did not develop and/or implement the means, procedures, and practices necessary for safe permit space entry operations, including, purging, inerting, flushing, or ventilating the permit space as necessary to eliminate or control atmospheric hazards: a. E E Robinson Park (Level Creek), 800 Peachtree Industrial Blvd. Sugar Hill, GA - An employee was exposed to asphyxiation hazard. The employer did not develop and/or implemented procedures for ventilating the confined space to control atmospheric hazard (oxygen deficiencies). The employer removed the manhole cover and purchased a flash light with the expectation that the employee would break the plain, on or about September 6, 2016.
