Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, 2016, the employer did not ensure that each forklift operator's driving performance was evaluated at least once every three years. The employer did not evaluate driving performance in the workplace after some drivers had received prior classroom training.
