29 CFR 1910.26(c)(3)(iii): The ladder base section was not placed with a secure footing: On or about 08/05/16, in the roof of the generator room area, employees were exposed to fall hazards while using a step ladder, that was being used as a non-self-supporting ladder, to access the roof of an adjacent structure.
