29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure: a) On or about June 29, 2016, and at times prior thereto; the employer did not ensure that employees performing servicing, maintenance and/or mounting/installing Die cutting blades were instructed in the purpose and use of the energy control procedure. Employees were exposed to struck-by, crushed-by moving parts, and/or amputation while performing work on Die cutting machines.
