29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Instances including, but not limited to, the following: a) Job site, 140 Commerce Drive, Freedom, PA: On or about June 1, 2016, the employer did not ensure that lockout devices were applied to energy isolating devices prior to performing preventive maintenance work on the Make On/Pull On machine, company identification number 003507, in that locks were not affixed to the air valves.
