29 CFR 1910.333(c)(3): The employer did not ensure that protective measures were provided before work by qualified persons on overhead lines was started. Protective measures, such as guarding, isolating, or insulating, shall prevent employees from contacting such lines directly with any part of their body or indirectly through conductive materials, tools, or equipment. On or about May 10, 2016, at 12281 U.S. 171, Noble, LA, employees were operating mechanical equipment before protective measures were in place regarding hazards that could arise if mechanical equipment made contact with an energized line, and the un-insulated boom of the mechanical equipment made contact with the primary overhead electrical power line of 14.4 kv.
