29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they did not utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device. a) On or about April 21, 2016, employees were performing die and embossing die changes on the Niagara E200 Presses without applying lock out devices to the electrical energy source on the presses for each employee conducting the change, thereby exposing employees to amputation hazards.
