29 CFR 1910.178(l)(4)(ii)(E): Refresher training was not provided to forklift operators after a condition changed that affected safe operation of the truck: a) At the Amercord facility in Lumber City: On or about April 14, 2016 and times prior thereto, employees were exposed to hazards not limited to tip-over and crush-by, in that LPG-powered forklift operators were not refresher trained prior to operating inside the poorly illuminated storage warehouse which included numerous unprotected drainage channels.
