29 CFR 1910.212(a)(3)(ii): Point of operation guards of machines were not designed and constructed to prevent an employee from injury by having a part of their body in the danger zone during operating cycles: (a) On or about March 30, 2016 and at time prior to that date, for the employees exposed to amputation hazards while operating a Cincinnati 75 ton press brake in the welding and fabrication room at 104 First Street Southwest, Cooperstown, North Dakota.
