29 CFR 1915.181(b): Before an employee was permitted to work on an electrical circuit, except when the circuit must remain energized for testing and adjusting, the circuit was not de-energized and checked at the point at which the work was done to ensure that it was actually de-energized. (a) On or about March 14, 2016; in CSER 4 located on the LCS 9; employees did not verify deenergization prior to installing the front plane on the SSABT (Solid State Automatic Bus Transfer) exposing employees to electrical shock hazards.
