29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative: The employer is failing to protect employees from electrocution hazards associated with live parts of equipment or the unexpected energization of electrical equipment when removing or installing Heating, Ventilation, and Air Conditioning equipment. This was most recently documented on or about March 1, 2016 at the Under Armour Store jobsite, located at 1843 Village West Parkway in Kansas City, Kansas, where employees were installing a new Heating, Ventilation, and Air Conditioning unit while the 480 volt electrical power was not locked out.
