29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight capacity of the truck and attachment combination at maximum elevation with load laterally centered: 101 North Main Street, Newark, Illinois, west building shop, Komatsu no. 45 counterbalance powered industrial truck - on or about March 1, 2016, modifications were made to fork attachments that affected vehicle's capacity and safe operation. Holes were drilled in forks to secure a job-made fifth wheel with pins/bolts for towing dump trailers. Forklift was not marked as required to identify this attachment configuration and show the approximate change in weight capacity. Employees were exposed to struck-by and/or crushed-by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
