29 CFR 1926.702(j)(1): Employee(s) was/were permitted to perform maintenance or repair work on equipment (such as compressors, mixers, screens or pumps used for concrete and masonry construction activities), where the inadvertent operation of the equipment could occur and cause injury, unless all potentially hazardous energy sources have been locked out and tagged. On or about October 1, 2015, at the above addressed job site, the employer permitted an employee to enter a concrete pump truck hopper to perform cleaning with a pneumatic chipping gun, where the hydraulic pressure had not been secured and locked out, which caused the auger to inadvertently start.
