29 CFR 1910.177(c)(2)(iv): The employer did not ensure that each employee demonstrated and maintained the ability to service rim wheels safely, including the performance of the following tasks, including, the use of the restraining device or barrier, and other equipment required by this section: b) BK Tires, Inc. at 3775 Puritan Way, Frederick, CO 80516: On or before July 16, 2015, the employer failed to assure that employees demonstrated and maintained the ability to service rim wheels safely, included continual use of the restraining devices required by this subpart. Condition exposed employees to projectile and struck-by hazards, and resulted in the fatality of an employee while he was inflating a 315/80R22.5 truck tire.
