29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder: a) At the jobsite, on or about 6/3/15: An employee was working on an extension ladder, approximately 16 to 18 feet from ground, removing a section of shingle from the eave of the roof, when he fell and died as a result of that fall.
