29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On or about 05/28/15 throughout the production areas; employer failed to protect employees involved in service and maintenance activities from amputation and caught between hazards, when periodic inspections of the energy control procedures of machines, such as but not limited to Muffler Hanger Winder, Muffler Hanger Cutter, Muffler Hanger Machine (vertical), were not conducted to verify accuracy of the procedures and compliance by Authorized employees.
