29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees to include but not limited to: a) On or about March 24, 2015, lockout procedures were not utilized during the cleaning of the Meyn Model #ME2080 (without conveyor line); known as the fat saver/sucker machine with the #189 vacuum tank. The electrical energy sources were not locked out.
