Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: A) Humanetics Innovative Solutions, Inc., located at 900 Denton Drive, Huron, OH 44839: On or about March 5, 2015, the employer did not ensure that equipment such as, but not limited to, the Enco Drill Press (Model 126-2240) located in the Ribs Department, was properly turned off/shut down while employees conducted activities such as, but not limited to, changing drill bits. Employees were exposed to injuries from unexpected start-up of the machine and energy release while electrical energy was not controlled prior to conducting work. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
