29 CFR 1915.89(c)(1): The employer did not ensure the use of a lock to prevent energization or startup, or the release of hazardous energy, before any servicing was started. F/V Alaska Mist, Coastal Dock, Factory Deck: On or about 4 February 2015 and times prior thereto, the Ryan 226 and Baader 415 Header machines had their cutting saws removed and were not locked out. This condition exposed employees to an amputation hazard.
