29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) do not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: (a) Throughout the facility; Operator and set-up personnel were not trained as authorized employees for the control of hazardous energy including, but not limited to: (i) Mechanical Press #PL115; The operator was not trained as an authorized employee for the control of hazardous energy, prior to affixing a lock out device, and (ii) Welding area; Operators and setup personnel were not trained as authorized employees for the control of hazardous energy prior to performing servicing and set up activities.
